Application Instructions

All application materials are due to Kelly Christianson, kchristianson@uwgcv.org, by April 12, 2024.

You MUST submit the following documents:

  • Application for appropriate activity
  • Required Budget Form or similar document with same information
  • List of current Board members (Non-profit organizations only)

ALL boxes in the application MUST be completed.

DUNS numbers are now obsolete. You MUST have a Unique Entity Identifier (UEI) number.

If your agency had a DUNS number, a UEI was assigned to your organization and you may be able to access it by visiting the System for Award Management (sam.gov) or the Federal Service Desk (fsd.gov). For more information, visit either website. If an agency does not have a UEI, it can be requested in, and assigned by, the System for Award Management (sam.gov). Without a UEI, applicant agencies cannot receive EFSP funding. Please note that the EFSP National Board did not assign UEIs to replace DUNS.

Programs must be established. These funds cannot be used as seed money or for start-up costs.
