ANNUAL PROGRESS REPORT

for
Fiscal Year 2021-2022 
(Period from 7/1/21 – 6/30/22)

Instructions

Who must submit an Annual Progress Report (APR)?

All agencies whose program(s) received funding from United Way of the Greater Chippewa Valley (UWGCV) during the past fiscal year (7/1/21-6/30/22) MUST complete and submit an APR by the established deadlines. Even if your agency’s program(s) only received funding for part of the year, the APR for 2021-2022 must be completed and submitted to UWGCV. UWGCV reserves the right to withhold funding if an agency does not submit the required documents by the established deadlines. 

When is the APR due?

The APR for 2021-2022 has two deadlines:   

  • Parts 1, 2 & 3 which includes Attachments A*- D (All initiatives), and J (Microgrants only) or K (Financial Stability only) are due on Monday, July 25, 2022. 

*Attachment A can also be submitted with Attachments E-I. Attachment A must be downloaded from the online form, completed for each program and then uploaded prior to submission. 

  • Part 4 which includes Attachments A*, E, F, G, H & I (financial documents) and is due on Friday, August 26, 2022

  • LOGIN HERE (If you already have a user name and password)
  • CREATE AN ACCOUNT (New users only - an email will be sent to you)
  • RESET YOUR PASSWORD (if your email has been used previously, you can reset your password)
  • LOGOUT (Be sure to log out once you have saved your draft or submitted your application)

How to Complete Each Part of the APR 

  • Complete Parts 1, 2, 3 AND 4 for EACH PROGRAM that receives grant funding from UWGCV. 
  • Be sure to click "SAVE DRAFT" frequently. You will be able to return and work on the application as your time permits. Be sure to click "SUBMIT" when completed. You will not be able to access your completed submission, but you will be sent a copy of your submitted APR. 
  • Incomplete APRs cannot be submitted. Do not leave any blanks. Enter NA if there is no applicable text. Enter "0" (zero) if the field has no numerical value.

Confirmation of Submission 

  • You will receive a confirmation email after each Part is submitted from Kelly Christianson, Lead Community Impact Director, along with a link to any of your attachments.
  • Be sure to make and retain a copy of your APR (including all attachments).

Review of Submitted Materials

Each APR will be reviewed by UWGCV staff and volunteers serving on the Advisory Council, Community Impact Committee, Finance Committee and Board of Directors. If clarification is needed or if questions arise during the review, you may be contacted and additional follow-up materials may be requested.
