I hereby release, indemnify, and hold harmless United Way of the Greater Chippewa Valley, the organizers, sponsors, and supervisors of all its activities from any and all liability in connection with injury, including any injury caused by negligence, in conjunction with this volunteer project. I likewise hold harmless from liability any person transporting me to or from any United Way activity.
In addition, United Way of the Greater Chippewa Valley has my permission to use my name and any photographs or videos taken for publicity purposes.
